24×7 PHC

🏥 24×7 Primary Health Centre (PHC) and NQAS

A 24×7 Primary Health Centre (PHC) is a first-level health facility mandated to provide round-the-clock healthcare services to the community. It plays a critical role in delivering comprehensive primary healthcare, especially for rural and underserved populations. A 24×7 PHC ensures availability of services at all hours, including outpatient care, emergency services, basic inpatient care, normal deliveries, newborn care, and essential diagnostic and pharmacy services.

However, continuous service availability alone is not sufficient unless it is accompanied by quality, safety, and patient satisfaction. To address this, the Ministry of Health & Family Welfare, Government of India, introduced the National Quality Assurance Standards (NQAS) under the National Health Mission (NHM). NQAS provides a nationally accepted quality framework for public health facilities including Sub Centres, PHCs, UPHCs, CHCs, and District Hospitals.

🎯 Purpose of NQAS in 24×7 PHC

The main objective of implementing NQAS in a 24×7 PHC is to:

  • Standardize healthcare service delivery
  • Improve patient safety and infection control
  • Ensure availability of essential drugs, equipment, and manpower
  • Strengthen documentation and record management
  • Improve patient experience and grievance redressal
  • Promote accountability and continuous quality improvement

NQAS shifts the focus from only service provision to service excellence by ensuring that every patient receives safe, effective, and respectful care at any time of the day or night.

📋 Scope of Services under NQAS for 24×7 PHC

A 24×7 PHC under NQAS is assessed for quality of services in:

  • Outpatient Department (OPD)
  • Inpatient Department (IPD)
  • Labour Room (Normal Delivery Services)
  • Emergency Services
  • Laboratory Services
  • Pharmacy Services
  • Immunization and MCH Services
  • National Health Programme implementation
  • Referral and transport services

Each service area is evaluated not only for infrastructure but also for:

  • Functional processes
  • Trained human resources
  • Standard treatment protocols
  • Patient safety practices
  • Biomedical waste management
  • Cleanliness and hygiene

🧩 NQAS Framework for 24×7 PHC

NQAS standards are organized under 8 Areas of Concern (A–H):

A. Service Provision – Availability and quality of clinical services
B. Patient Rights – Respect, privacy, information, and grievance redressal
C. Inputs – Human resources, drugs, equipment, and infrastructure
D. Support Services – Lab, pharmacy, laundry, and housekeeping
E. Clinical Care – Standard treatment protocols and rational drug use
F. Infection Control – Biomedical waste management, hand hygiene, sterilization
G. Quality Management – Committees, internal assessments, and audits
H. Outcome – Patient satisfaction and service utilization

These standards help a 24×7 PHC systematically identify gaps and implement corrective actions.

🌟 Benefits of NQAS Certification for 24×7 PHC

NQAS-certified 24×7 PHCs experience:

  • Improved cleanliness and infection prevention
  • Better patient trust and satisfaction
  • Organized record keeping and reporting
  • Enhanced staff discipline and teamwork
  • Improved maternal and newborn outcomes
  • Recognition and financial incentives under NHM
  • Stronger referral linkages

NQAS also motivates healthcare workers to follow standard operating procedures (SOPs) and adopt a culture of quality and safety.

🏆 Importance of 24×7 PHC in Public Health System

24×7 PHCs are critical for:

  • Reducing maternal and neonatal mortality
  • Managing emergencies at the local level
  • Decongesting higher facilities (CHC/DH)
  • Ensuring continuity of care
  • Implementing national health programmes effectively

With NQAS, these PHCs evolve from service points into quality-assured health facilities.

Quality is not dependent on manpower alone—it depends on commitment, systems, and teamwork.
